California School Physical Requirements: What Parents Need to Know

Whether your child is entering kindergarten, starting middle school, or joining a sports team, California has specific health requirements that must be met before they can attend. Here's what you need to know.
School Entry Health Examination
California law requires a health examination for all children entering first grade (or kindergarten in some districts). This exam, completed on the Report of Health Examination form, must be done within 18 months before school entry. It includes a physical exam, vision screening, hearing screening, dental assessment, and review of immunization records.
Required Immunizations for School
California requires the following vaccines for school entry: DTaP (diphtheria, tetanus, pertussis), Polio, MMR (measles, mumps, rubella), Hepatitis B, and Varicella (chickenpox). For 7th grade entry, a Tdap booster is additionally required. California does not accept personal belief exemptions — only medical exemptions are valid.
Sports Physicals
Student athletes must have a pre-participation physical exam before trying out for or participating in school sports. This exam evaluates heart health, musculoskeletal fitness, and overall readiness for physical activity. It should be completed annually.
When to Schedule
Don't wait until the last minute. We recommend scheduling school and sports physicals 4–8 weeks before they're needed to allow time for any follow-up vaccinations or referrals. Late summer appointments fill up fast.
What to Bring
Bring your child's immunization records, any required school forms, insurance card, and a list of current medications or health concerns. If transferring from another state or country, bring all previous medical records.
